My understanding is that wine that does not ordinarily effervesce or foam can be made to foam by the addition of spices (as described in the passage cited in the question). The reference in the passage to the pouring out of wine is symbolic, as it also is elsewhere in Scripture (for example, Revelation 14:10, Revelation 16:19, or Jeremiah 25:15) of God unleashing His wrath. The fact that the wine is foaming indicates the increased or intensified degree of that wrath.
